A doctor-patient relationship with one of the Stewart doctors is ONLY achieved by a "prospective patient" filling out and signing appropriate Stewart paperwork and forms including but not limited to intake forms, HIPAA forms, informed consent forms, and other forms as required by Stewart and the individual doctor and then that "prospective patient" meeting with the Stewart doctor face to face at the office of that Stewart doctor or at a hospital where the Stewart doctor has privileges and that "prospective patient" being accepted as a "patient" by that individual Stewart doctor. The acceptance of which must be in writing in the chart as well as orally to the "prospective patient" by the Stewart doctor.
The Stewart doctors appear on TV and the internet. They are doctors and not TV professionals. They give their "best efforts" and try hard to provide accurate and truthful information, Content without errors or omissions. TV and internet are inherently fraught with mistakes and misstatements. TV and the Internet have three basic problems. First is the time constraint of the TV or the internet. With TV or Internet videos or live broadcast, there is not enough time to go into details and specifics about Content. Secondly, with TV and on the internet the Stewart doctors are asked by the station to speak in "people speak" instead of technical chiropractic or medical terms that the average viewer or listener may not understand. The Stewart doctors give their "best effort" to accurately report and discuss all topics and render information and opinions as Content. Third, live TV waits for no man, meaning the doctor is often put on the spot having to come up with something to say. Again, Stewart doctors use their best efforts to provide accurate Content. In giving their "best effort" and speaking in people speak, and being put on the spot, often mistakes are made including but not limited to misstatements, errors, omissions, malapropism, slip of the tongue misinformation, hereafter know as Mal-Content. We apology in advance for those mistakes. Stewart nor it's doctors individually or collectively shall be held liable for any Mal-Content including but not limited to misstatements, errors, omissions, malapropism, slip of the tongue, inaccuracies or delays or for any actions taken in consequence as we make no warranties implied or stated as to the correctness of the content.
All persons receiving any Content or viewing or listening to broadcast of Content, or talking to a Stewart doctor in person receiving Content, are duty bound by the following: Should you have issue with Content then You, as Claimant, must first notify Stewart Chiropractic, by certified mail at 1349 Wolfe St, Jacksonville, FL 32205 of any misstatements in Content or Mal-Content. Your making a claim of misstatement, omission or Mal-content constitutes an agreement by and between you the Claimant and Dr. Cameron Stewart, DC, Stewart Chiropractic, their doctors, providers, and employees hereafter known as Stewart Chiropractic. Then, Stewart Chiropractic has 30 business days to respond to you in writing by certified mail at the address stated in your complaint of our response to your claim of misstatement, omissions or Malcontent. Should we agree that the misstatement, omission or mal-Content needs correcting, then our remedy is within 30 days of our written response to you for the Stewart Chiropractic doctor to retract and correct the misstatement, omission or Mal-Content either on the air OR on our website. Should the misstatement of Content, Omission or Mal-Content be retracted and restated correctly or the corrected Content statement be published on our website for 30 days, then NO other remedies may be sought after and your retraction by Stewart Chiropractic or publishing corrected Content on our website shall constitute good consideration for the General Release as stated below by and between you and Stewart Chiropractic.
No representation is made by Stewart Chiropractic or the Content therein or otherwise that the quality of the Chiropractic services to be performed is greater than the quality of Chiropractic services performed by other Chiropractors. Results vary case to case and no guarantee or warranty of outcome is made. Charges vary based on services rendered. Additional charges may be incurred for related services which may be required in individual cases. Some persons with certain insurance cannot receive free services. If that is YOUR case, then YOU are responsible to inform the Stewart Chiropractic doctor in writing, prior to receipt of services, that YOU cannot receive FREE services. Should you receive FREE services without informing our doctor in writing then YOU are responsible for the outcome and indemnify and hold Stewart Chiropractic and the Stewart Chiropractic doctor harmless. Videos on this site contain persons that are not Stewart Chiropractic patients.
Most insurance today has a deductible and a per visit percentage co-pay. It is your responsibility to call your insurance company prior to service being rendered to determine how much deductible and co-pay or percentage co-payment you have. We are committed to providing you with the best possible medical care at the lowest possible cost. Please understand that the payments on your bill are considered a part of your treatment. The following are statements of our financial policies: Payment for all visits are due at the time of treatment. Stewart Chiropractic providers are NON-PARTICIPATING in all insurance networks. You are responsible for your entire bill. We will be happy to provide you with a statement that you will be able to submit to your insurance carrier for reimbursement. Stewart Chiropractic, any of its providers, or employees will NOT enter into any dispute with your insurance comply. If coverage problems arise, you will be expected to deal directly with your insurance company, adjuster, or agent. Before any radiology testing or lab work is performed or ordered, it is the patients’ responsibility to check with their insurance company for a laboratory or facility that is covered under their insurance plan. If a suit, action, or other proceedings of any nature is instituted in connection with any controversy arising out of our financial policy (agreement), or to interpret or enforce any rights under this agreement, the prevailing party may recover reasonable attorneys’ fees and cost. All insurance companies today now have disclaimers that state verification of coverage is not a guarantee of payment. Therefore YOU are responsible for your bill.

Some persons may need additional X-rays or imaging depending on their individual case. Some patients are not allowed to receive reduced fee chiropractic or medical services. This offer also does Not apply to Personal Injury cases or Workers Comp cases. For all services, results vary in individual cases.
OUR OFFICE POLICY IS THE PATENT OR ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICES OR TREATMENT WHICH IS PREFERRED AS A RESULT AND WITHIN 72 HOURS OF RESPONDING TO THIS AD FOR FREE SERVICE. General Release: For good consideration, in the form of a retraction or restatement of Content, or from Stewart Chiropractic, publishing corrected Content on its website for 30 days, the receipt of good consideration which is hereby acknowledged, the claimant party, Releaser, does hereby forever remise, release, acquit, satisfy, and forever discharge Stewart Chiropractic, of and from all manner of actions, causes of actions, proceedings at law or administrative law action or equity actions, suits, debts, covenants, contracts, controversies, agreements, promises, claims and demands whatsoever, which said claimant ever had, now has, or which any personal representative, successor, heir or assign of said releaser Claimant, hereafter can, shall or may have, against Stewart Chiropractic, by reason of any matter, cause of thing whatsoever related to the Claimants' assertion of misstatement, Mal-Content or Omissions, from the beginning of time. Claimant hereby agrees to indemnify Stewart Chiropractic and pay Stewart Chiropractic's cost of defense of said General Release and include reasonable attorneys fees. This General Release shall be construed and the validity, performance, and enforcement thereof shall be governed by the Laws of the state of Florida.
